Should I guess in a negative-marking exam?

Only after considering the official scheme and your evidence from valid elimination. Blind guessing can reduce net marks.

When should I skip a question?

Skip when the route is unclear or time cost exceeds your practised threshold; mark it for a later pass if appropriate.

Change only when you find new evidence such as a missed condition, calculation error or misread option.
